Report Application Changes
MNsure certified assisters and certified broker support staff can help individuals with reporting changes.
How to Report Changes
Use the information below as a guide to determine when to report different types of application changes and where they should be completed.
Health plans sold through MNsure: Changes must be reported within 30 days and completed using the "Edit Application" button through the consumer's MNsure account.
Medical Assistance (MA): If anyone in the household has MA, report changes within 10 days by contacting the county or Tribal office.
MinnesotaCare: If everyone in the household has MinnesotaCare, report changes within 30 days by contacting the Department of Human Services (DHS) at 651-297-3862 or 800-657-3672.
Mixed-Eligibility Households: Visit the Mixed-Eligibility Guide on MNsure.org for information and complete details.
Additional information can be found in Report a Change on MNsure.org.
Qualified Health Plan Enrollees (QHP)
Enrollees need to report all application changes. The changes could impact the enrollment in their plan and their eligibility for premium tax credits and cost-sharing reductions, if they applied for financial assistance.
Medical Assistance and MinnesotaCare Enrollees
Enrollees should follow instructions on the DHS website for how and when to report changes. MinnesotaCare members can call the MinnesotaCare information line at any time to get automated information on the status of their case, the status of their renewal and instructions to pay their MinnesotaCare premium over the phone. This document provides a consumer the information they need to have available when using the automated line and the phone prompts.
